The following restaurant inspections with critical violations were conducted by Richland Public Health between Aug. 11 and 15:
● Mansfield Reformatory Preservation Society/Scofield Café, 100 Reformatory Road, Mansfield, Aug. 11. Presence of live insects, rodents and other pests (critical). Observed flies during inspection. Correct by Aug. 15. Direct connection between the sewage system and a drain originating from equipment in which food, portable equipment or utensils are placed (critical). Observed improper air gap on Coke machine located in Scofield Café. Correct by Aug. 15.
● Mansfield Restaurant Inc./Southside, 948 S. Main St., Mansfield, Aug. 11. TCS foods not being cold held at the proper temperature (critical repeat). Observed Advent Edge refrigeration reading 53 degrees. Will recheck within 24 hours. Correct immediately. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical repeat). Observed milk that was opened in walk-in cooler that didn't have date marking. Correct immediately. Working containers of poisonous or toxic materials not properly labeled (critical repeat). Observed chemical buckets that are not properly identified, located at front counter.
● Sol Froyo, 2170 Walker Lake Road, Ontario, Aug. 11. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Observed cut strawberries at 53 degrees on customer self-service cold holding unit. Person in charge (PIC) states strawberries were cut within the last 30 minutes; placed strawberries into smaller portions in walk-in cooler. Discussed cooling cut fruit to 41 degrees before placing in customer self-service cold holding unit. ● McDonald's - Richland Mall #3350, 666 Lexington Springmill Road, Mansfield, Aug. 12. Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected during inspection). Observed tea, lemons and butter not covered.
● American Legion Post 535, 77 Bell St., Bellville, Aug. 12. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected during inspection). PIC signed policy.
● Pretzelmaker (Richland Mall), 2208 Richland Mall, Space #C-8, Mansfield, Aug. 12. Person in charge did not ensure that employees observed foods as they were received (critical). Employee states temperature of TCS food are not being taken upon delivery to ensure they are delivered at required temperatures. Correct by Aug. 22. Person in charge did not ensure employees are properly cooking TCS foods and/or using thermometers as required (critical). Employee stated hot dogs are not checked for proper temperature following cooking in oven before being placed in hot holding. Correct by Aug. 22. PIC unable to demonstrate knowledge of food temperatures and times for safe cooking (critical). Correct by Aug. 22. PIC unable to demonstrate knowledge by having no critical violations (critical). Education provided for each critical at time of inspection.
Food employee(s) did not wash hands when required (critical). Observed food employee remove glove after picking something up off the floor, however, employee did not wash hands prior to donning new gloves. Correct by Aug. 22. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed pretzel dough rising on trays placed on racks at front counter to be uncovered/unprotected. Improper reheating of food for hot holding (critical, corrected).
Observed ready-to-eat, TCS food (cream cheese) at only 63 degrees after an hour and a half of heating. Employee unable to bring cream cheese to proper heating temp of 135 degrees within two hours or less. Employee voluntarily discarded 3 pounds of cream cheese. Food contact surfaces not easily cleanable (critical). Observed scissors that are used to open bags of food items to not be easily cleanable. Please obtain scissors that are able to be fully taken apart to effectively be cleaned by Aug. 22.
● JB's Drive In, 265 S. Delaware St., Lexington, Aug. 15. Quaternary ammonium sanitizing solution at incorrect temperature, concentration and/or water hardness. (critical repeat, corrected during inspection). Observed a bucket of sanitizer solution at 100 ppm. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected). Observed opened containers of coney sauce and sloppy joe that had been held for more than 24 hours in the refrigerator without a date mark.
● Der Dutchman Restaurant, 720 Ohio 97 W., Bellville, Aug. 15. Food employee(s) did not wash hands when required (critical, corrected during inspection). Observed food employee touch hair and proceed to attempt task without washing hands. Food package(s) received in poor condition (critical, corrected). Observed can of diced tomatoes with dent along top seal. PIC removed can. Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (critical repeat, corrected).
Observed working bucket of quat sanitizer at 0 ppm. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Observed dirty utensils in clean utensil storage. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Observed dried food residue and green buildup in meat slicer. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Observed blades of knives in clean storage to have dried residue. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Observed dried spillage on clean food pans. TCS foods not being cold held at the proper temperature (critical, corrected). Observed pie cooler to have ambient temperature of between 47 and 48 degrees.
Observed multiple cream-based pies in unit to be 44 degrees. PIC moved all pies in unit to another cooler. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected). Observed chocolate milk in service area to not be date marked. Working containers of poisonous or toxic materials not properly labeled (critical, corrected). Observed working container of soap to not have label of common name. Presence of live insects (critical). Observed multiple flying insects. Correct by Aug. 22. Manual wash solution not being maintained at 110 degrees or above (critical, corrected). Observed manual warewashing water to be at 104 degrees.
● Fast Eddie's Pizza LLC, 59 Main St. RR, Bellville, Aug. 15. Person in charge not ensuring proper cooling (critical). Cooling logs showed that cooling process was not beginning when food registered 135 degrees. Correct by Aug. 15.
● Happy House Inc., 201 E. Main St. #3, Lexington, Aug. 15. Person in charge not ensuring proper cooling (critical, corrected during inspection). Observed fried rice out on table cooling was below 135 degrees. Handwashing sink not accessible (critical, corrected). Bag stored in hand sink by mop sink. Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected). Roast pork and sauce in two-door make table across from cook line were uncovered.

● Wooster Outdoor Center, 4654 Pleasant Hill Road, Perrysville, Aug. 15. Water source and/or hot water capacity not sufficient (critical). Observed no hot water at hand sink. Manager stated that water heater is not operating properly. Facility is not operating for 14 days and only doing functioned weekend events. Provide hot water by next event or only handle paper and plastic utensil ware and limited food prep.
